The Firm

House Call Solutions is a management consulting firm focused on the rapidly emerging arena of community-based chronic care management.  House Call Solutions was originally formed in 2005 to provide high-quality business development and management services to clients in the burgeoning medical house call industry.  Growing with our clients, today we provide programmatic solutions to the financial and clinical challenges chronically-ill patients present to a range of players in the health care industry.  

Our flagship product, the House Call Program, is a comprehensive primary care medical home visit program.  The House Call Program enables an organization to humanely serve frail, medically complex, low-mobility seniors in a high quality, cost-effective manner while delivering strategic and financial benefits to the sponsor. 

Our experience, skills and capabilities have positioned us as an innovator in chronic care management programming, enabling House Call Solutions to respond to client needs for assistance with other community-based patient management programs, including transitional care and patient-centered medical home: 

  • Transitional Care - often thoughts of as a “bridge to home”, these programs provide the right level of short duration, in-home medical care to ensure a coordinated and seamless transition from hospital or SNF to home, reducing the possibility of further decline in functional and/or health status, as well as inappropriate or redundant utilization of health services.

  • Patient-Centered Medical Home – An increasingly prevalent area of interest, PCMH leverages the PCP’s direct and trusted relationship with patients, coupled with a depth and breadth of clinical training across body systems, to assess an individual’s health needs and to tailor a comprehensive, coordinated approach to care that spans health condi­tions, care settings and providers.

House Call Solutions has developed viable business models for a variety of programs serving a range of organizations and communities.  Our client base includes health systems, managed care organizations, home health agencies, community service organizations and medical groups.  Though diverse, all have one goal in common:  to meet the needs of under-served, chronically-ill populations in a fiscally responsible manner while returning benefits to program sponsors.



Brent T. Feorene
President

Brent T. Feorene brings to clients 18 years of experience consulting to the healthcare industry.

The focus of Mr. Feorene's experience is in identifying and addressing the strategic and management issues impacting physicians, medical groups, and hospital-physician programs and relations.

Clients have included hospitals and health systems, academic medical centers, physician organizations, MSOs, community service organizations and home health agencies.

Mr. Feorene originally formed House Call Solutions to provide high-quality business services to clients in the burgeoning medical house call industry. Growing with his clients, he is on the leading edge of the rapidly emerging community-based chronic care management market. Mr. Feorene has developed viable business models for a variety of programs benefiting a range of organizations and communities. Though diverse, all have one goal in common: to meet the needs of under-served, low-mobility populations whiling ensuring strategic and financial benefit to program sponsors.

As president of House Call Solutions, Mr. Feorene provides a comprehensive approach to his clients. He is involved in all aspects of client engagements, from planning and development to support, education and management. He is committed to helping you serve the needs of your patients/clients.

Active as an advocate and thought leader in the rapidly evolving community-based chronic care management arena, Mr. Feorene is on the Board of the American Academy of Home Care Physicians (AAHCP) and serves on the Executive Committee as Treasurer. He was requested to chair an ad hoc AAHCP committee tasked with developing a new book on advanced house call program management; serving as editor and authoring several chapters (target publication Spring 2010).

Prior to forming House Call Solutions, Mr. Feorene spent 11 years with Medimetrix Consulting, a Cleveland-based, national healthcare consultancy. As a senior staff member, he led strategic business, program/service line, and high-level operations management engagements with physicians, medical groups, health systems, managed care organizations and pharmaceutical companies.

Mr. Feorene holds an MBA with highest honors from the Weatherhead School of Management of Case Western Reserve University, with dual concentrations in finance and strategy. A respected presenter and author, he has written and spoken on a variety of strategic and management issues impacting healthcare, most recently on current community-based care management initiatives, including house calls, patient-centered medical homes and transitional care/bridges to home programs.


Kelly Johnson
Consultant

Kelly M. Johnson, R.N., M.S.N., L.N.C., brings over ten years of diverse physician practice and hospital operations experience to House Call Solution clients.

In her role as a consultant with HCS, Ms. Johnson provides clients with significant operational expertise in the implementation, management and re-engineering of house call programs, whether affiliated with a health system, home health agency, MCO or part of a private medical practice.

She works closely with provider and administrative staff to ensure effective, efficient operations that support high levels of quality care and patient service, expense control and revenue maximization. Ms. Johnson is an expert in house call program accreditation by The Joint Commission and offers HCS clients leadership in developing operational standards, policies and procedures that are compliant with The Joint Commission accreditation standards.

Ms. Johnson is the founding manager of Norton House Calls, a five-year old house call program affiliated with Norton Healthcare in Louisville, KY. Norton Healthcare, Louisville’s leading health care system and third largest private employer, is a not-for-profit system which includes five hospitals. Currently, Ms. Johnson is Director of Risk Management for Norton Physician Services, a medical group with more than 380 employed medical providers at over 90 locations. Ms. Johnson retains a consultative management relationship with Norton House Calls.

Ms. Johnson holds a Master of Science degree in nursing from Bellarmine University and holds a certificate in Legal Nurse Consultant.
